📘 What Is the Medicare Diabetes Prevention Program?

The Medicare Diabetes Prevention Program (MDPP) is a structured lifestyle change program covered by Medicare Part B for eligible participants.

The program focuses on practical, sustainable changes that support long-term health, including:

  • 🥗 Healthy eating habits you can maintain over time
  • 🚶 Safe, doable physical activity
  • ⚖️ Weight management, when appropriate
  • 🧠 Building daily habits that support lasting wellness

This is not a diet program and not a medication program. It’s designed for real life and focuses on progress, not perfection.

📅 What You’ll Receive

🟦 6-Month Core Program

  • 💻 16 structured group sessions (virtual)
  • 📝 Step-by-step guidance on nutrition, movement, and lifestyle habits
  • 🤝 Motivation and accountability from a trained lifestyle coach

I have tried to lower my weight and A1C for several years, but I plateaued. After joining Melita’s T2 online group in September of 2025, I’ve been able to utilize the program’s strategies (e.g., portion control, replacing sweets with healthier choices) to reduce my weight by approximately 7%. This, in turn, helped lower my A1C from 6.2% to 5.9%. While I have always been active and regularly exceeded 150 minutes per week – the guidance provided by the T2 Program gave me a new perspective. While there is no silver bullet to attack diabetes, by using a multi-prong attack and focusing on small steps, success can be obtained.

This program, Prevent T2, has been a very comprehensive program to assist in developing lifetime skills to lower my glucose and body weight.

 I originally was diagnosed with prediabetes with an A1C of 6.2. My most recent blood labs indicate I am no longer prediabetic with an A2C of 5.2 I have lost 30 pounds and wish to continue until I reach a healthy weight goal.

 Each Chapter focuses on skills needed to enhance my chosen diet. I see them as tools to use in my journey to healthier living.    

Having a group zoom meeting once a week and accountability weigh in, kept me on track.                                                                                         

Melita was an encouraging coach. She showed an authentic concern for those who signed up for the program. My goal was to learn to keep my glucose levels from spiking. With proper diet, exercise and small habit changes I believe I have met that goal. I feel so much better. I am no longer out of breath. My vision of my future is brighter. I still have some weight to lose, but I know I can achieve that goal and keep it off.

✅ Who Is Eligible?

Medicare Part B covers the Medicare Diabetes Prevention Program if all of the following apply:

  • 🧪 Within the past 12 months, you had one of the following:
  • Hemoglobin A1c between 5.7% and 6.4%
  • Fasting plasma glucose between 110–125 mg/dL
  • 2-hour plasma glucose between 140–199 mg/dL
  • 📏 BMI of 25 or higher (23 or higher if you’re Asian)
  • 🚫 No prior diagnosis of type 1 or type 2 diabetes
  • 🚫 No End-Stage Renal Disease (ESRD)
  • 🚫 No previous participation in the Medicare Diabetes Prevention Program
